What Is a Frenum?

The frenum is a naturally occurring soft tissue band located in three primary places in the mouth:

  • Beneath the upper lip (labial frenum)
  • Beneath the lower lip
  • Under the tongue (lingual frenum)

While this tissue is normal and typically causes no issues, it can sometimes be too short, too thick, or too tight, which may pull on the gums, restrict tongue movement, or cause gaps between teeth.

Common Reasons for a Frenectomy

A frenectomy is often recommended when a problematic frenum causes one or more of the following:

Speech difficulties

A short or tight lingual frenum (often referred to as “tongue-tie”) can limit tongue mobility and lead to articulation challenges or delayed speech development in children.

Gum recession

When the frenum pulls on the gum tissue—particularly between the lower front teeth—it can cause the gum to recede over time, exposing the tooth root and making it more susceptible to sensitivity and damage.

Spacing between teeth

A frenectomy is typically quick and gentle. Before we begin, the area is numbed with a local anesthetic to ensure you do not feel any pain or discomfort during the procedure. For children—or anyone who needs additional relaxation—we often offer nitrous oxide (laughing gas) or even moderate sedation, for a more relaxed experience.

Using precise instruments and, in many cases, a soft tissue laser, your periodontist will carefully release or remove the tight frenum fibers. Laser techniques allow for a minimally invasive approach with minimal bleeding, reduced need for sutures, and a faster recovery. The entire procedure typically takes only 15 to 30 minutes, and you will receive detailed post-operative care instructions. Most patients heal quickly—often within a few days—and notice immediate improvements in comfort, movement, or spacing.
